The state runs four regional mental health institutes, in Memphis, Nashville, Chattanooga, and Bolivar, which handle much of the public inpatient psychiatric load.

• community mental health centers, some now certified as community behavioral health clinics, carrying much of the public caseload,
• the regional mental health institutes and hospital psychiatric units for inpatient and acute hours,
• academic medical centers and children's hospitals in Nashville and Memphis, where most child psychiatry sits,
• addiction and opioid treatment programs, which are unusually plentiful here,
• private psychiatry and telepsychiatry groups, and Veterans Affairs behavioral health in Memphis, Nashville, and upper East Tennessee.

Tennessee has carried one of the heaviest opioid burdens in the country for years, and the response has built out a dense network of medication assisted treatment and opioid treatment programs, especially across the east and the rural counties. For a PMHNP student, that is an opportunity: substance-use psychiatry is often easier to secure here than child work, and it pairs naturally with medication management and with the psychotherapy training your program requires.

A substance use rotation in Tennessee can carry real weight in a lifespan plan. For a Tennessee rotation, the current program decides whether telehealth or simulation is permitted and how either is classified, supervised, documented, and applied. Confirm the current program rule and any applicable state requirements before relying on either format.

Because PMHNP is an across-the-lifespan credential, your hours have to reach children and adolescents, adults, and older adults, plus supervised psychotherapy in at least two modalities. Child and adolescent psychiatry is the tightest to place, and in Tennessee it clusters in the academic and children's-hospital settings of Nashville and Memphis, which is a real distance for a student in the mountains or the rural west.

Geriatric psychiatry is somewhat easier to find, through long-term care, geriatric clinics, and rural hospitals. We treat the child and adolescent block as the piece to lock down first and build the rest of the calendar around it.

Tennessee is a restricted practice state. A nurse practitioner practices with physician supervision, and prescriptive authority, including for the controlled psychiatric medications a PMHNP relies on, is tied to that supervising physician and to a formulary, with a personal DEA registration required for any controlled substance.

In practice, this shapes who precepts you more than what a good preceptor knows how to do. A psychiatrist is often part of the arrangement, whether as your direct preceptor or as the supervising physician behind a PMHNP preceptor. Your preceptor is generally a board-certified PMHNP (PMHNP-BC) or a board-certified psychiatrist, and your program sets the final requirements.
